DOT Physical Exam for Pennsylvania CDL 2026: Medical Certification Guide

The physical exam itself follows federal commercial-driver medical standards. Pennsylvania’s role is to maintain the medical qualification on the driver record and tie it to the CDL or CLP status.
Use a National Registry medical examiner
Commercial drivers who are not excepted from the medical qualification rules must be examined by an FMCSA-certified medical examiner listed on the National Registry. The examiner determines whether you meet the applicable physical qualification standards and how long the certificate should remain valid.
Self-certification still matters
Pennsylvania requires commercial drivers to declare the type of commerce in which they operate. The medical-document requirement depends on the category and the work. Complete the state self-certification accurately rather than copying another driver’s category.
The June 23, 2025 change
PennDOT implemented electronic medical-certification integration on June 23, 2025. FMCSA now transmits examination results and medical restrictions from the National Registry directly to PennDOT. PennDOT says Medical Examiner’s Certificate information is accepted only electronically from FMCSA and is no longer accepted in person or by email.
Why record status matters
PennDOT warns that a driver who should have a current medical certificate can be marked not certified and can face downgrade of the CDL to a non-commercial credential if the medical status is not current. Renew before expiration and follow up promptly if the record does not update.
Practical checklist
- Confirm the examiner is currently listed on the FMCSA National Registry.
- Bring medication and medical-history information requested by the examiner.
- Keep your own copy of the MEC and examination paperwork.
- Verify the medical status on your Pennsylvania record, especially near a permit, renewal, or employment deadline.
