DOT Physical Exam Michigan CDL 2026: Medical Certification Guide

Michigan's medical-certification process changed materially in 2025. For drivers who need a federal Medical Examiner's Certificate, the important workflow is now electronic: the certified examiner reports the result to FMCSA's National Registry, and Michigan receives it through that system.

First decide which driving category applies

Every CDL applicant or holder must self-certify the type of commercial driving they perform. The core distinction is interstate versus intrastate and excepted versus non-excepted. Most drivers operating in non-excepted interstate commerce must maintain a current federal medical certificate. If your work fits an exception, the medical requirement can differ, so choose the category based on the work you actually perform rather than the route you hope to drive someday.

Michigan switched to electronic MEC reporting

Michigan says that as of June 14, 2025 it checks FMCSA's National Registry when issuing a CLP or CDL to a non-excepted interstate driver, and Medical Examiner's Certificates are accepted electronically from the National Registry. Certified medical examiners must report examination results to the registry by midnight of the next calendar day after the exam.

What you should do after the physical

• Use an examiner listed on FMCSA's National Registry of Certified Medical Examiners.
• Make sure your name, date of birth, license information, and state are entered accurately at the exam; mismatched data can delay the electronic match.
• Check your medical-certificate information in Michigan Secretary of State Online Services rather than assuming the transmission succeeded.
• If your examiner reports a problem, work with the examiner and FMCSA promptly instead of waiting until your CDL privileges are affected.

Do not let the certificate expire

Michigan warns that if a required medical certificate or variance expires without a valid replacement, commercial driving privileges can be removed. If your type of commercial operation changes, update your self-certification before the current medical certification expires. Changing from interstate to intrastate may also require a corrected CDL showing the K restriction.

Medical certification is not the same as a drug test

The DOT physical can include urinalysis for medical purposes, but it is not the federal DOT drug test. Employer drug and alcohol testing and the FMCSA Clearinghouse operate under a separate compliance system.
