California CDL medical requirements in 2026: July 27 electronic reporting for interstate drivers, intrastate paper options, National Registry examiners, and status checks.
California's medical-certification process changed again in 2026. Effective July 27, 2026, interstate non-excepted commercial-driver examination results are submitted electronically to DMV through FMCSA's National Registry. Intrastate commercial drivers may still use California's paper submission channels under the current DMV rules.
Interstate Drivers
For interstate commercial drivers who require medical qualification, use a medical examiner listed on FMCSA's National Registry of Certified Medical Examiners. Under California's July 27, 2026 implementation, the examiner submits the Medical Examination Report and Medical Examiner's Certificate electronically through the National Registry.
California states that the examiner must submit the result within 24 hours and that DMV must post the medical examination result to the driver record within one business day under the electronic interstate process.
Intrastate Drivers
California's current medical page distinguishes intrastate commercial drivers from the new interstate electronic flow. Intrastate drivers may still submit the MER and MEC in paper form through the accepted DMV channels, including in person, mail, or the Virtual Field Office.
Use the Correct Examiner
Federal interstate examinations must use a National Registry examiner. California also imposes specific examiner requirements for certain passenger-transport certificates such as school bus and related credentials. Use the current DMV medical page for your exact commercial operation.
Certificate Timing
Commercial drivers should not assume every medical certificate lasts two years. A certificate may be issued for up to the normal federal maximum, but the examiner can use a shorter period when a medical condition requires more frequent follow-up. Your own expiration date controls.
Medical vs. Drug Testing
The DOT physical determines medical qualification. Federal drug-and-alcohol testing is a separate employer compliance system. Completing the medical examination does not replace pre-employment, random, post-accident, or other testing obligations that may apply under Parts 40 and 382.
